Registering at the practice
When you register at Capelfield Surgery, you will be asked to complete three forms. The first is the GMS1 form which enables us to officially accept you as a patient and obtain your medical records from your previous GP. The second is our own medical questionnaire which will help us understand your medical history whilst we wait for your medical records to arrive. The third is a questionnaire on your alcohol consumption.
You will also be asked to bring x1 proof your identity and x2 recent proof of your address when registering.
Proof of identity can include most types of photo identification including drivers licence, provisional licence or passport.
Proof of address will include most official documents that are addressed to you in your name at your home address within the last 3 months. Please note that we will require two different types of document eg. a gas bill and a telephone bill.
If you need to be seen by a GP whilst your application is being processed, you can be seen as a temporary patient.
Print off a registration form, fill it out and bring it in with you on your first visit to the practice.
